Printed on properly sized A5 cardstock. Forms can be ordered blank or filled out (personalized).
Personalized version will be filled out on a period German typewriter and include appropriate divisional stamp and commanding general signature. 
 
When ordering personalized (filled out), please provide necessary information - name, unit and rank as well as the desired date/place of award. If no place is given it will be filled out as "Im Felde" (in field) which was very common.
