A5 certificate issues by the Air Force command (Luftwaffe). Called "Urkunde für Angehörige von Gefallenen" - Certificate for Relatives of the Fallen.
As all Krause Papierwerke certificates it can be customized (personalized) with any name you desire. It will be filled out on a period German typewriter and stamped & signed accordingly. 
Example in the picures is filled out for fihghter ace Werner Mölders and signed by Hermann Göring himself!
When ordering personalized version please mention name, rank, unit and date of death. Optionally date of document issue (otherwise I'll make it 1-4wks after unfortunate death).
