Originally embossed stamp - here printed on both sides, and Adolf Hitler's facsimile signature come also with blank form. 
As with all certificates you may choose to personalize it and you will get it filled out on a German WW2 SS typewriter (Groma) with a name, rank, unit and award date of your choice!
